Are you a nurse who loves working with expecting and new mothers along with young children? Do you enjoying working in the community? The Norfolk Department of Public Health is seeking a highly motivated Registered Nurse to become a member of our district’s BabyCare program. This program primarily provides nurse case management services to low income pregnant women at high risk of poor birth outcomes to ensure healthy babies and the tools needed to be effective parents. Specific program areas include: case management of medical services, pregnancy screening and referrals, prenatal care education, child safety education, and child development education. Collaborates and participates with local organizations and boards to promote positive prenatal health. Plans and coordinates patient services with department staff, support services and community agencies to assure continuity of care and desired outcomes. This nurse will foster opportunities to reduce health disparities. This nurse will independently provide professional public health nursing care with individuals, community groups, partnerships, coalitions and other key players to achieve program objectives related to improving birth outcomes by ensuring pregnant women and infants receive all the services they need within the District. Assists clients to access services through health department, private providers and community agencies. Documents interventions according to program and agency protocol. Service setting is primarily within the community, in the homes of clients, but includes some on-site clinical services. Works with supervisor and Babycare team to plan, implement and evaluate services to address Healthy People objectives, VDH and District priorities. This nurse may also assist and work in the health department clinics and will also participate in agency or city emergency preparedness and events.
The successful candidate will have knowledge, skills and abilities related to principles and practice of professional nursing, public heath/community health nursing, home visiting, maternal and child health and experience in administering and interpreting childhood immunizations. The candidate will have the ability and skill to work well independently and as a team member. Candidate must have effective written and verbal communication skills. Possess significant skills and ability to teach and work effectively with community groups, establish partnerships and collaborate to achieve program objectives. Candidate must have strong skills and abilities to critically process the needs & resources of clients, families, and the community along with the ability to teach individual and groups, to work effectively with persons with complex socio-economic situations and diverse cultural backgrounds. Effective MS Office computer skills are a necessity. Candidate must possess an unrestricted RN licensure in Virginia or a compact state and valid driver’s license.
Prefer BSN or equivalent training and experience. Advanced public health practice or community experience is desirable. Experience in case management and bilingual English/ Spanish skills are a plus.
